New Patient Information

To streamline your initial visit, we ask that you arrive 15 minutes early with your driver’s license and insurance card in hand so that you can fill out patient-registration forms.

We reserve 60-minute appointment slots for adults for a complete exam that includes the following:

• X-rays
• Imaging
• Gum disease screening
• Periodontal charting

During your exam, your provider will walk you through their findings. From there, they will work closely with you to create a dental plan designed to meet your unique oral health goals.

Children’s appointments are shorter. As long as the child allows it, the same procedures are completed.
